Lapses at multiple stages resulted in Odisha train accident last month

Commission of Railway Safety has given its report regarding the Odisha train accident that took last month. As per the report, there were lapses at numerous levels in the Signal & Telecommunication (S&T) department which resulted in this deadly accident. Further, the report state that lapses in signaling also resulted in this accident.
